AHEC Practice Support Consultants help you:

Practices involved in AHEC Practice Support Services learn to:
- Use patient feedback and clinical performance data to facilitate practice quality improvement
- Adopt and implement care protocols based on national clinical guidelines
- Use clinical decision support tools
- Ensure patients are cared for by their preferred provider
- Involve all providers and staff as part of a team that delivers patient care
- Communicate with patients using secure email and web portals
- Offer same-day appointments and expanded hours
- Provide or arrange for patients’ preventive, acute, chronic and end-of-life care
- Monitor care patients receive from specialists, hospitals, home health, etc.
- Generate and exchange electronic health information
- Reach out to patients with chronic diseases to ensure symptoms are well-controlled
- Teach patients to manage chronic conditions
- Partner with patients and families to set and achieve goals
- Base care on the needs and preferences of patients and families
- Involve patients actively in selecting treatment options
- Communicate with patients in a language they understand, taking cultural differences into account
